| This is for those who are curious about what we had to complete for our international adoption process. First of all, we had to complete a series of paperwork required by our adoption agency and for our state. This included - Background checks with the local and state police department, searches with the department of family services, medical reports for my husband and I as well as a letter of good health for each of our four biological children from their pediatritian. We also had to submit a four page autobiograhy for each of us. List of our paperwork for our agency in particular. Letter of petition to adopt written to the CCAA Homestudy report - I-171H My Birth certificate Pats Birth certificate Copies of the childrens birth certificates Statement of health from my bio childrens pediatrician Marriage certificate Medical form for each parent Financial statement Employers letters Police Report for each parent Criminal background check by State Department of family services, records check Passports (copies of picture and signature pages) Photos of prospective parents, home, and family life Reference letters 2 from family members 3 from non-family members |
